Incident Investigation Overview

Upon learning that a work-related injury or near-miss has occurred with an employee, supervisors must submit an incident report as soon as they are able, allowing EH&S to launch an accident investigation. The goal of these investigations is to collaboratively determine the root cause(s) and suggest corrective actions that can help prevent future incidents from occurring.

Members of EH&S routinely assess the incident reports received from the campus community and perform follow-ups with supervisors and their employees. Appropriate and timely response to incidents helps maintain the effectiveness of the program, which leads to a safer learning environment for the campus community. Prompt reporting is also essential for OSHA compliance and OSU Risk Management purposes.
